Innovation (CMMI) released its “<a href="https://www.cms.gov/priorities/innovation/data-and-reports/2024/rtc-2024">2024 Report to Congress</a>.” The report details the role that CMMI plays in the health care industry, as the Centers for Medicare and Medicaid Services (CMS) is the largest payer for health care in the country and CMMI is responsible for testing ways to improve the quality of care while reducing federal spending on Medicare, Medicaid, and the Children’s Health Insurance Program (CHIP).</p> <p>The report notes that over the past decade, CMS has tested more than 50 payment and care delivery models, resulting in a change in the way “we pay for care in the United States to focus more on quality, clinical outcomes, and patient experience.” Elizabeth Fowler, CMMI Director, notes that insights gained from those models have influenced broader healthcare practices, as many private payers have adopted CMS-inspired approaches.</p> <p>For models that demonstrate success at improving quality, reducing cost, or both, CMMI has leveraged other pathways to scale such models – beyond certification and formal expansion. Some of the different scaling approaches may include incorporating successful elements of models into CMS programs and successor models.</p> <p>CMMI issues biennial reports to Congress and since the last Report in 2022, CMMI has made progress, including announcing nine new models. Each model is designed to deliver on five strategic objectives: driving accountable care, advancing health equity, supporting innovation, addressing affordability, and partnering to achieve system transformation.</p> <p>CMMI also notes its commitment to reducing health disparities and increasing impact on care for underserved populations, including implementing payment adjustments in models to bridge the gap between historic spending levels and actual costs of caring for underserved beneficiaries, as well as to support participants who serve a higher proportion of underserved populations. The report notes that the number of safety net providers participating in Medicare models has “significantly expanded.”</p> <p><strong>Strategic Accomplishments</strong></p> <p>CMMI highlighted some of its strategic accomplishments in the report, including redesigning and launching the Accountable Care Organization Realizing Equity, Access, and Community Health ACO REACH) Model as another avenue for Medicare beneficiaries to establish accountable care relationships with their providers. ACO REACH participation grew from 52 ACOs and about 350,000 beneficiaries in 2021 to 132 ACOs and 2.1 million beneficiaries in 2023.</p> <p>Additionally, the Making Care Primary (MCP) Model was launched in eight states, igniting the move to value-based payment for primary care clinicians and equipping the primary care providers with tools to form partnerships with health care specialists.</p> <p>CMMI also announced the voluntary Cell and Gene Therapy Access (CGT Access) Model for states and manufacturers to test whether a CMS-led approach to developing and administering outcomes-based agreements for cell and gene therapies improves the Medicaid beneficiaries’ access to innovative treatment and improve their health outcomes, all while reducing health care costs to states.</p> <p>CMMI also discussed the importance of partnerships in the report, citing the IBH Model, which focuses on Federal-state partnership in support of the integration of primary care and behavioral health. IBH is a state-based model that focuses on community-based behavioral health practices that treat Medicare and Medicaid beneficiaries.</p> <p><strong>Model-Specific Highlights</strong></p> <p><em>Accountable Care Models</em></p> <p>The ACO REACH Model focused on underserved populations, emphasizing health equity plans and innovative payment approaches while the Comprehensive Primary Care Plus (CPC+) Model reduced emergency department visits, but highlighted challenges in achieving net Medicare savings.</p> <p><em>Disease-Specific Models</em></p> <p>The Kidney Care Choices (KCC) Model improved patient outcomes by increasing education and emphasizing care coordination while the Bundled Payments for Care Improvement Advanced (BPCI Advanced) demonstrated savings in surgical episodes, but required adjustments for medical conditions.</p> <p>Prescription Drug Models focused on affordability and access, including initiatives like the Medicare $2 Drug List Model.</p> <p>CMMI also mentioned success with State and Community-Based Models, including the Maryland Total Cost of Care Model, which served as a blueprint for new initiatives like AHEAD, which emphasizes multi-payer alignment.</p> <p><strong>Recommendations and Future Directions</strong></p> <p>In conclusion, CMMI plans to focus expansion on models that are successful on both cost savings and quality improvements while emphasizing collaboration with state governments and private payers to align on payment models and care delivery strategies.
